Case Study Concerning The Well-Being Of Mrs. Nancy Jamison

The following document is created to discuss a case study concerning the well-being of Mrs. Nancy Jamison, a 67 rear-old retired woman who is suffering from basilar infiltrates consistent with congestive heart failure. There were a few steps that could have been omitted when Mrs. Jamison arrived to the RediCare located at the local mall. The on-duty care provider there should have checked her insurance and administered her to the appropriate hospital immediately for prompt care. It was very time-consuming to transport he r to other hospitals. Immediately following the medical history and medication list that was provided by the computer system, and the physical assessment was performed, Mrs.Jamison should have been disposition to City Heart Institute cath lab. For prompt attention at arrival, Dr. Miller should contact the cardiologist on call at the City Heart Institute, so that appropriate measures will be applied upon arrival. Other observations regarding the care provided for Mrs. Jamison were the lack of prompt decision making skills, lack of considering the time, lack of administering proper medication for release of pain and various symptoms and lack of researching the insurance information so that Mrs. Jamison is disposition to the correct hospital appropriate for the symptoms she was having.In this case, Mrs. Jamison was provided with the care that she needed. However, the entire process was very time-consuming and much could have been omitted. The entire process took approximately two hours before she received the proper care. It does not take long to have a heart attack or for the problem to worsen. Jana, at RediCare, suggests that Mrs. Jamison is administered to the Marysville Community Hospital, but Mrs. Jamison should have been immediately transported to the City Heart Institute cath lab which would have saved some time.There are several locations in Philadelphia that provide services for various needs: 1. Coronary Care: Hospital of the University of Pennsylvania. The hospital has a special unit for Coronary Care. Located at 3400 Bruce Street in Philadelphia. 2. Trauma Care: Hahnemann University Hospital. Hahnemann University Hospital is a 541-bed academic medical center at Broad & Vine Streets in Philadelphia, Pa. The hospital is a tertiary care institution that specializes in trauma services. 3. Burn Care: Temple University Hospital.The hospital has a Severe Burn Care Unit that specializes specifically on the burn. Located at 3401 North Broad Street 400 Carnell Hall in Philadelphia. 4. Complicated Obstetrical Care: Temple University Hospital. Temple Care University Hospital is a major provider to obstetrical services. Located at 3401 North Broad Street 400 Carnell Hall in Philadelphia. 5. Premature Infant Care: The Children’s Hospital of Philadelphia. The hospital has a Neonatal Intensive Care Unit (NICU) that provides services for complex problems for premature babies who are in need of intensive medical attention.Located on 34th Street and Center Boulevard in Philadelphia. Biography of Franklin Delano Roosevelt Essay

The 32nd president of the United States, Franklin Delano Roosevelt, accomplished many things during his three terms in office. Being the only president ever elected to more than two terms, it is easy to suppose that he was a well-rounded president. His leadership, success in getting programs passed, management of economic and domestic policy, foreign policy, and role as Chief Party Leader all contribute to an overall grade of an A for his presidency. First off, as the symbol of the country and the presidency, Franklin Delano Roosevelt gets an A. He showed great character and leadership with his thirty â€Å"fireside chats† on the radio. With these, he was able to soothe the thirty five million people listening. It was successful too, in that, after his first in which he assured the safety of keeping money in a reopened bank, confidence was restored and the banks began to open up. His ability to master the press contributed strongly to his overall public opinion and showed he had good character. Also, in WWII conferences such as Casablanca and Teheran, Roosevelt represented the country well in his efforts with the Allies to stop Nazi Germany. His leadership of the country and his character are the reason that he was the only president ever to be elected to four consecutive terms as the leader of the country. Particularly with the New Deal, FDR was rather successful as a policy and legislative leader. With the nation in the midst of the Great Depression, there was a lot of pressure on the president to take immediate action. Beginning with â€Å"the first hundred days,† Roosevelt shut down the banks and met with Congress. He was able to get 15 major bills approved regarding the economy in attempts to get it back on track. With such turmoil happening in the country, FDR was very efficient in getting bills passed by Congress, which is why he deserves an A. Though the actual success of each program may be questioned, his quick action and role as a policy and legislative leader was executed well. The same is true for when the United States became involved in WWII. This situation again demanded quick action and Roosevelt pulled through by passing various acts such as the Lend Lease Act and Neutrality Acts. Though again these may not have always had the best outcomes, his initiative and f air thinking are what gets him an A for this category. FDR’s most noted domestic policies while in office were those of the New Deal. With a nation pressuring the president for change, Roosevelt developed a plan called the New Deal. The principal acts were done during his first hundred days, as mentioned earlier, in attempts to achieve his goals of relief, recovery, and reform. Regarding the economy, Congressed passed the Glass-Stegall Banking Act which established the FDIC, a very important act after the crises leading up to the Great Depression. By stopping the bank runs, he was able to steer the economy in the right direction, creating a corporation that is still used today. In addition, under his presidency, the Civilian Conservation Corps (CCC) was established, giving jobs to young men while at the same time conserving both human and natural resources. With the young men being able to send the money back to their families, it was a way to decrease unemployment and boost the economy. Another important issue was his repeal of t he Prohibition through the 21st amendment. This gave much needed revenue to the country and provided employment. Despite these successful acts of domestic policy, Roosevelt was not always so brilliant. The Agricultural Adjustment Administration, one of the New Deal agencies, increased unemployment, frustrated farmers, and was eventually deemed unconstitutional by the Supreme Court. His intentions and theories to end overproduction were good, just not executed well. Another of Roosevelt’s reforms is Social Security. This is perhaps one of the most controversial of his policies. Some loved it and some hated it. In fact, the benefits of this administration are still debated over today with people calling into question the longevity of its existence. One problem that Roosevelt seemed to face was his long-running battle with the Supreme Court. Consisting of older aged, ultraconservative men whom Roosevelt had not appointed, the Supreme Court was something the president wanted to change. To fix it, he asked Congress to permit him to add up to 15 justices to the Supreme Court. This attempt to â€Å"pack† the court cost Roosevelt severely in the eyes of the public. It was his away of going around the Constitution in order to be able to keep more of his programs and agencies from the New Deal. In total, despite also several failures, many of FDR’s domestic policies were good. They helped relieve some of the stress set on by the depression. However, that’s all they did – relieve. His policies and the New Deal did not cure the country of the Great Depression, only temporarily masked the problems with only of his agencies still existing after his presidency. For these reasons, I give Roosevelt a B for his domestic policy. In the beginnings of a second world war, Roosevelt had important foreign policies that were crucial to the United States’ welfare. With an attempt to end the Depression, Roosevelt was initially going to send a delegate to take part in the London Conference. However, upon a second look, he decided that international agreements about the value of the dollar might restrict him in domestic recovery. This begins a trend toward increased nationalism and eventually to the United States’ views on isolationism. Though we are unable to tell what might have come out of the conference, it seems like a poor decision for Roosevelt to leave America totally on its own from other countries. Along with this idea of isolationism, through the Tydings-McDuffie Act, the United States freed the Philippines (after 12 years). This act was more beneficial for the United States than the actual freed country because of less foreign competition for sugar, as well as fewer people competing for jobs. Overall, it did help the U.S.; however, it also kind of abandoned the Philippines with not-so-nice economic terms. In contrast to these inward-looking acts, Roosevelt also created the Good Neighbor Policy with Latin America. He focused on consultation and nonintervention in hopes to unite the Western Hemisphere against any powers on the other side of the world. This was a smart move after withdrawing ties from Europe and Asia. It was a good idea that ended up being very successful for him and Roosevelt ended up with the title of â€Å"traveling salesman for peace.† In such fear of war, Congress passed the Neutrality Acts so as not to be brought into the violence like had been done in WWI. However, Roosevelt later changed these Acts to mean that an arms embargo be established for both Loyalists and rebels. As far as Roosevelt’s foreign policies go, this one was really bad. So scared of war, they basically aided in Franco’s taking over of the republican government of Spain. This in turn led to the strengthening of the European dictators, leading to the second world war. For this reason, Roosevelt’s grade goes down. Then, in 1941, neutrality had its meaning changed again. Really, it was thrown out the window with passing of Lend-Lease Act. The reasoning behind it was very understandable. Arm the other fighting democracies with weapons to defend against Hitler, or allow them to fall and have the U.S. face them alone. Knowing the risk, and after many debates, it was finally approved. It also set up the United States for war production when they did fight. However, the Lend-Lease Act ultimately was what brought them into the war once they had to start using armed Destroyer ships to take them to Britain. Roosevelt’s idea was good, however the consequences were bad. Later in the war, Roosevelt met with Churchill and Stalin for the Teheran Conference. Here, the three agreed upon a plan to attack Germany from the East as well as from the West. This was an incredibly important part of Roosevelt’s foreign policy since it was the beginning of the end for Nazi Germany. The Allies were able to recover France from the Nazis and continue their fight with Germany. Though Roosevelt died a month before V-E Day, his position as Commander in Chief for the past twelve years caused the United States and the Allies to defeat Germany. Though nobody is perfect, Roosevelt was able – through all decisions, good and bad – to stop the Nazis and ultimately save Europe. For these reasons, Roosevelt gets an A as Commander in Chief. Franklin Delano Roosevelt was strong in communicating with the public was well as dictating foreign and domestic policy. For this reason, his party grew during his presidency and so he deserves an A as Chief Party Leader. He started the â€Å"FDR Coalition† for his first term to get votes. Throughout his successes in the beginning, he was able to change many Republicans into Democrats, strengthening the Democratic Party for years to come. In addition, he also aided more Democratic candidates in all areas of government into office. With Roosevelt in office, the party grew both in voters as well as representation in Congress and other branches of government. His role as Chief Party Leader was done well and that is why he gets an A. Franklin Delano Roosevelt came into office at first having to deal with the struggles of the Great Depression. He was then later faced with the challenges of World War II, attempting to protect the United States as well as all Democratic countries from Nazi takeover. Health Safety All employees have personal responsibility in ensuring their own and others health, safety, and welfare at work. We are required to read and apply organisational policies and procedures regarding health and safety, undertake necessary training, wear personal protective equipment (PPE) provided, use safety equipment and safety devices where required and report any hazardous conditions to the line manager or Health and Safety Officer. 6. Confidentiality All information concerning customers and/or their families must be treated  with the upmost respect and all employees have an obligation to protect this information. Confidential information must not be disclosed to a third party either formally or informally (gossip). The sharing of information may be necessary for continuation of good care and support of customers (e.g. between other employees or professions). If employees are asked to disclose information about a customer and are concerned that doing so may breach confidentiality then clarification should be sought via the line manager. 7. Whistle blowing If employees need to raise concern about a risk, malpractice or wrongdoing that affects others such as customers, other staff members, the company or the public then the company’s whistleblowing procedure should be followed. The first stage of this procedure would be to raise concerns with the employee’s line manager either in writing or through a meeting. If the issue involves the line manager or if the employee has already raised their [emailprotected] ?U: concerns with the line manager and feel that they have not been adequately addressed then the second stage of the procedure should be followed. At this stage, issues may be raised either in writing or through a meeting with a Director. If stage 2 has been undertaken and the employees concerns are unresolved or if they feel that the matter is so serious that it should be investigated by the Chief Executive immediately then concerns should be raised either in writing or via a meeting. Any employee raising concerns under the whistleblowing procedure has the right to remain anonymous. Bvi As a Community Home Carer, my role is to provide individualized care and support to people with social care needs so that they may continue to live in their homes and in the community. A care plan is in place for each customer that explains their specific needs and what care they require from us. My duties/responsibilities may include: Assisting customers with washing, bathing, personal hygiene, dressing, toileting, continence and catheter care. Meal and drink preparation and help with feeding where necessary. Helping customers to comply with their prescribed medication and fill out Medication Administration Records (MAR). Supporting customers  with cleaning, laundry, shopping and other household duties as required. Helping customers with mobility skills, transferring them safely where required (e.g. from bed to chair) and helping them to use aids and personal equipment. Providing end of life care. Documenting the care given at each visit in the Daily Support Notes (DSN). Liaising with other carers and health care professionals to ensure continuity of care. By carrying out these duties to a high standard and conforming to all policies, procedures and guidelines, customers will receive a quality service that will help them to maintain as much independence as possible in a safe environment within their own homes. The safeguarding carried out by carers is essential in protecting the health, wellbeing and human rights of vulnerable adults, and ensures that they live free from harm, abuse and neglect. It is our duty to report any safeguarding concerns to the appropriate person(s) so that action may be taken to protect the wellbeing of service users. Bvii a) Following best practice within my work role helps to safeguard vulnerable adults living in the community and ensures that they receive a quality service that promotes health and wellbeing. It is important that I stay within my job role, follow policies and procedures, keep up to date with changes in policies and legislation and keep up to date with training to develop my skills in the workplace. It is also important to work as part of a team, passing on skills where necessary (e.g. to new staff members) and gaining feedback from colleagues. Good communication skills are essential between team members and other professions involved in the care of the service user as this enables continuity of care. b) Failing to carry out the requirements of my role by not following policies, procedures and legislation and not complying with regulatory bodies endangers the health and wellbeing of service users and may put other team members at risk. Not following manual handling procedures, for example, would put me and the service user at risk of injury and not wearing personal protective equipment  when necessary may put me and others at risk of infection. Poor practice may also result in complaints from service users, disciplinary action and in severe cases it may lead to me losing my job, being prosecuted and/or prevented from working in the health and social care setting. Bviii As a Community Home Carer I must follow best practice within my work role and work to the company policies and procedures that mirror laws and legislation. There are also external factors which influence the standards that I must adhere to in my work such as the Care Quality Commission (CQC) essential standards, Skills for Care’s Common Induction Standards (CIS) which are the standards people working in health social care need to meet before they can safely work unsupervised (e.g. manual handling, first aid, health and safety training), the Health and Care Professions Council (HCPC) codes of practice and ADASS’ ‘Safeguarding Adults: A National Framework of Standards for good practice and outcomes in adult protection work’, which sets out a framework of best practice standards. Government initiatives also have an influence on the roles of carers such as ‘Our Care, Our Health Our Say’ which sets out policy goals to make health and social care ser vices that are designed around the patient rather than the needs of the patient being forced to fit around the service already provided. ‘Putting People First’ also aims for a personalised approach to the delivery of adult social care. Bix a) Two different representative bodies which influence my area of work are the care quality commission and the health and care professions council. b) The role of the care quality commission (CQC) is to regulate, inspect and review all adult health and social care services in the public, private and voluntary sectors in England and make sure that they are meeting national standards. These services include hospitals, care homes, GPs, dentists and services in the home (e.g. home carers). The Health and Care Professions Council (HCPC) are a regulatory body set up to protect the public by keeping a register of health and care professionals that meet their standards for training, professional skills, behaviour and health. If professionals fail  to meet these standards then they may be removed from the register and prevented from practicing. The HCPC replaced the General Social Care Council (GSCC) which was abolished in 2012.

Search for my Tongue Essay Sujata Bhatt tells us about the difficulties that she has speaking with a new tongue when her old tongue starts to rot away in her mouth with her new tongue pushing it out of the way and trying to take over. Your mother tongue would rot, rot and die in your mouth until you had to spit it out. This means the author has stucked between two languages and the new language (English) is making her lose mother tongue (Gujarati). Having two tongues this poet feels that she is totally confused and makes her to forget her mother tongue while she speaks English. She also tried to think and dream both languages at the same time but she couldnt. She has dreamt in Gujarati and transliterated into English. At the end of the poem her feelings changes a bit because she describes over the night her confidence grows back even stronger than before, but while she dreams it grows back, stump of a shoot grows longer, grows moist, grows strong veins, it tries the other tongue in knots. This means she highlights the difficulties being part of two cultures. The dominant culture is always the mother tongue (her Gujarati culture is always the influences of the American lifestyle). The shape of the poem has divided into three parts:  ? First part of the poem explains her conflict with loosing her  mother tongue and learning a new foreign tongue.  ? Second part of the poem is written in Gujarati (mother tongue) and explains her fear of loosing her identity.  ? Third part of the poem is translated in to English and focuses on her determination to retain her Gujarati culture. The poet includes the Gujarati as an indication of the strong link between language and culture. This shows us that she tries to use the both languages at the same time in her dreams. The central part of the poem is looks different because it has written in Gujarati and transliterated into English. I think the poet included this Gujerati script and its phonetic prescutation underneath as an indication of the strong link between language and culture and possible to you to realise how difficult it would be in a foreign country and speaking in a foreign language.  Fundamentally, one image links this whole poem is that a flower. She compares her mother tongue to a flower that grows (a symbol of beauty and life), like a flower grows the foreign language also grows but her mother tongue is stronger eventually. This is called Extended Metaphor. I think the poet used this extended metaphor in order to compare the differences and influences of two languages. The list below describes some of the ways in which her mother tongue is compared to a plant. The poet uses both negative and positive images in describing her mother tongue. Sujata Bahatt thinks that foreign tongue has most powerful effect than Gujarati but Gujarati culture overcomes the influences of the American style and still makes the mother tongue strong.  In conclusion, I believe that I have learnt a lot about the culture and traditions of an immigrant. The writer feels that she has confused in between two languages. She feels her mother tongue is being lost in her mouth and foreign tongue is becoming more frequently used, this is making her uncomfortable. At the end of the poem, I feel that she gives us an inside view of what it must feel like to be in a foreign country and speaking in a foreign language.
